- have it approved by an official of the provincial education department before the activity takes place;
- keep a copy of the approved form at the school; the official keeps the original.
What the Schedule 1 form asks for
- school and governing body details, and the purpose of the activity (day or overnight, number of days, accommodation and catering);
- the number of learners (boys and girls), educators and other adults (male and female), and the names of the accompanying adults;
- the cost per person and the support strategy for learners whose parents cannot afford to pay;
- transport company, roadworthy certificate, driver and substitute driver details (licence, professional driving permit, insurance) and the support system en route;
- attached: a full itinerary, and an invitation letter if the school was invited.
It is signed by the principal and the governing body chairperson, then approved or refused by the Head of the provincial education department or a delegate.
