Regulation D.9 lists the mandatory fields for all real-time electronic submissions to the HCR:

Last name, first name, date of birth
Address (street address or PO Box), city or town, postal code, e-mail address
Team name, Team Division, and Team Category (AAA, AA, A, B, C, 1, 2, 3, etc.)
Role on Team/MHA (Player, Goaltender, Coach, Bench Staff, Safety, or Official)
For officials: referee or linesperson certification, season of qualification and criminal record check
For coaches: level of coach certification, season of qualification and criminal record check
For safety personnel: level, expiry date and criminal record check
Registration date, name of Minor Hockey Association
Affiliate status, Import status, Tryout status
Release status and Release date
Active season (for example 2026-2027)

Registration data must be kept in a protected and secure manner. Members retain ownership and control of their own data in the HCR, and Hockey Canada makes no commercial use of it unless it agrees in writing with the Member or Members.
