- School Sport: for students competing up to national level within the school or National Sporting Organisation pathway.
- National Sport: for athletes competing up to national level. SIA's FAQ says this is for athletes aged 18 and over.
- International (Virtus): required for Virtus World and Regional Championships and Global Games, and a prerequisite for Paralympic sports classification. The Virtus international form also provides national-level eligibility.
An athlete may need to submit at least two forms if they progress from national to international competition.
