NFHS Rule 4-2-3: if the referee or coach suspects a wrestler has a communicable skin disease or another condition that makes participation inadvisable:
- The coach must provide current written documentation from a physician (as defined by the NFHS or the state association) stating the condition is not communicable and won't harm an opponent.
- It must be furnished at the weigh-in for the dual meet or tournament.
- The only exception: a designated on-site meet physician examines the wrestler immediately before or after the weigh-in.
- Covering a communicable condition does not make the wrestler eligible.
4-2-4: a designated on-site meet physician may overrule the diagnosis of the physician who signed the release, either way.
4-2-5: documentation for a non-communicable condition (birthmark, eczema, psoriasis) is valid for the whole season. It may need re-evaluation if the condition becomes secondarily infected.
Use of the NFHS Skin Lesion Form was a 2025-26 point of emphasis. Many state associations publish their own version, and some set who may sign it.
