There are two levels, and a flagrant foul must be confirmed by instant replay before it is ruled.
- Flagrant 1: contact against a player that is unnecessary. The offender is charged a personal foul and the team a team foul. The fouled team gets two free throws and the ball at the free throw line extended. A player is ejected for a second flagrant foul in the same game.
- Flagrant 2: contact that is unnecessary and excessive. It carries the same two free throws and possession, and it is an unsportsmanlike act, so the offender is ejected.
If the fouled player is injured and cannot shoot, the coach picks a replacement shooter (for a Flagrant 1, one of the other four players on court). The shot clock is reset to 24 seconds after any flagrant foul.
