Complete a Hockey Canada Injury Report Form for every injury during a sanctioned event such as a game or practice, whatever its severity.
- Fill in the form fully and sign it, including the insurance portion. Incomplete forms are not processed. Page two (the physician's statement) must be completed by a medical doctor, not a physiotherapist.
- Email it to the BC Hockey office at injuryclaims@bchockey.net within 90 days of the incident. If the form is not received within 90 days, the claim will not be processed.
- Invoices and receipts can follow the original report, up to 52 weeks after the injury date. Claims received after the 90-day period may affect payment.
Hockey Canada is the secondary insurer: file with any extended benefits or other insurance first, then send the explanation of benefits with invoices and receipts to BC Hockey. Out-of-country players must buy separate primary medical insurance for the program to apply.
