- Section 1: the athlete (or a parent or guardian if under 18) completes the athlete details and agrees to the terms in the Informed Consent form.
- Section 2: the Athletics Athlete Evaluation Sheet (Provisional Athletics Classification Form). Any GP, physiotherapist or specialist can complete it, and only the pages relevant to the athlete's condition need completing.
- Section 3: the medical diagnostics section. The family can provide a medical letter from a doctor confirming the health condition, or ask a doctor to complete pages 8 and 9. Physiotherapists cannot diagnose.
- Submit the application to classification@athletics.org.au.
Australian Athletics then issues the provisional classification. If you are unsure about a complicated case, the Para Athletics Coordinator can be reached at classification@athletics.org.au.
